HBI’s Home and Family Assessment Team’s mission is to determine the recovery environment at home and if necessary, recommend follow-up services which may include outpatient counseling and rehabilitative services.

Inpatient Stabilization

HBI can help make the proper referral for inpatient admission when the patient’s mental health assessment reveals a need for 24-hour observation and stabilization for either psychiatric or chemical dependency issues.

We will help you determine if you need to step down from an inpatient program to intensive outpatient treatment, but not requiring 24-hour observation and monitoring.

Medication Management

HBI Psychiatric Unit consists of a team of Board Certified Psychiatrists and Practitioners to help those in need of medication to stabilize their symptoms or provide relief from depression, anxieties, or chronic psychiatric conditions. Your therapist can make the proper referral for you to see a qualified psychiatrist.

Part of wrap-around services, counselors may recommend a PSR worker to provide a client needed psychosocial rehab and basic living skills training (i.e., self-care, education, occupational concerns, and compliance with treatment including medication).

OPEN™ Outreach Psychiatric Evaluation Program

The HBI OPEN Program is designed to do assessment and follow-up on patients who are in group homes, nursing homes, private residences, and assisted living facilities and unable to attend therapy visits; also work with and provide support to home health agencies and caregivers.
